Serenity guides you through the procedures for preparing the documentation you must present in order to receive the products free of charge through the National Health Service. In the Download Area you can download the Urination Diary; filling out this diary will save you time and ensure that already have on hand the correct documentation when you go to your primary care doctor.

2) Fill out the Urination Diary

In the Urination Diary, mark down the frequency and volume of the urine, onset of urination, appearance of the urine, etc.).

3) Go to your primary care doctor and request a referral for an examination at your reference N.H.S.

Bring your completed Urination Diary to your primary care doctor who will make an initial evaluation of your incontinence.

4) Make an appointment with your N.H.S. and request authorization for the prescription

Go to your reference N.H.S. If stabilized incontinence is certified, go to the Prosthetics Department with your doctor's referral authorizing the prescription for the free incontinency supplies. To find out which N.S.H. or District you should contact, just fill out the form below.

5) Interview with Nurse

At the N.H.S. Prosthetics Department, nurses will help you identify which supplies are best suited to your needs.
